The Synthes 357.389 drill sleeve is a guided instrument component designed to precisely control drilling during orthopedic surgery, particularly when placing or preparing for trochanteric fixation nails. The dual-diameter (8.0 mm external, 4.0 mm internal) configuration allows it to accommodate a 4.0 mm drill bit (or similar) while being stabilized through an 8.0 mm outer fit in the protection or guide sleeve. This design helps reduce lateral wobble and ensures accurate alignment of the drill bit with the pathway required by the nail system.

At approximately 164 mm in length, the sleeve is long enough to reach through soft tissue, guide over the nail or insertion pathway, and maintain an adequate working distance for both the surgeon and the drill.
