The Shiley 5.5PEF cuffless pediatric tracheostomy tube is intended for pediatric patients who require an airway but for whom a cuff (inflatable balloon) is not indicated. With an inner diameter of 5.5 mm, outer diameter of ~7.9 mm, and a tube length of about 46 mm from tip to flange, this size is toward the larger pediatric range but still smaller than typical adult tubes. The cuffless design allows air to leak around the tube, which can be favorable in spontaneous breathing patients, enabling natural airflow, easier phonation if vocal cords are functional, and generally reduced risk of pressure injury to the tracheal mucosa associated with cuffed tubes.

In terms of construction and comfort, the 5.5PEF includes design features intended to reduce irritation and support stoma care. The flange is soft, transparent, and designed to allow visualization of the underlying skin so that signs of redness or skin breakdown can be detected early. The flange has recessed areas to reduce contact pressure against the skin, which helps reduce irritation from contact or securing devices (e.g. tape, trach ties). Material is non‑DEHP in the newer Shiley pediatric lines, which is an advantage in reducing potential exposure to certain plasticizers, particularly for younger patients. The standard 15 mm connector makes it compatible with typical ventilator or humidifier circuits, speech valves, etc.

Clinically, the 5.5PEF is used when patient anatomy (airway diameter, stoma size) supports that size without causing undue trauma, and when goals include minimizing airway resistance while avoiding the risks associated with cuffed tubes. Because there is no cuff to seal, leaks are expected; this may influence ventilator settings (if ventilated), humidification, and management of secretions. It may be especially useful in patients who are spontaneously breathing, being weaned, or in whom phonation/speech is desired. The trade‑offs include that absence of sealing via cuff means less protection against aspiration and possibly more work of breathing if there is excessive leak. Regular monitoring for tube fit, skin condition at stoma site, patency of the lumen (secretions), and comfort are essential to avoid complications.
