The Shiley 6UN75R cuffless flexible tracheostomy tube is designed for adult patients who do not require cuff sealing—i.e. in scenarios where airway sealing is not essential, or where spontaneous / non‑ventilated breathing, speaking, or airflow via the upper airway is desired. By being cuffless, it avoids pressure exertion on the tracheal mucosa, and allows for airflow around the outer cannula — a beneficial feature during weaning or when using speaking valves or capping trials. The flexible construction helps the tube conform to patient anatomy and tolerates some movement without undue irritation.

Because the tube includes a reusable / removable inner cannula, clinicians can remove or clean the inner lumen as needed without removing the entire tracheostomy tube. This is useful in managing secretions, maintaining lumen patency, and reducing the need for full tube exchange when obstruction occurs. The beveled tip design aids insertion, especially in percutaneous or interventional tracheostomy contexts, where passage through tissue and tracheal stoma must be smooth. The soft flange and biocompatible materials are intended to reduce pressure on the stoma site and adjacent skin, improving patient comfort and reducing risk of skin breakdown.

In practice, the 6UN75R would be selected in a patient who is stable, has minimal risk of aspiration, and for whom a sealed cuff is not essential (for example, during long-term airway maintenance with spontaneous breathing). It is not ideal when positive-pressure ventilation demanding a tight seal is required. When the inner cannula is removed (e.g. for cleaning), the airway remains patent via the outer cannula and peritubal airflow. However, because the tube is cuffless, care must be taken regarding aspiration risk, humidification, airway protection, and secretion management.
