The Shiley 8FEN is a large adult‑size tracheostomy tube designed to provide both secure ventilation/airway protection and flexibility for the patient to use their upper airway when possible. The “fenestrated” design means that once the cuff is deflated and when it’s clinically appropriate, air can flow up through the fenestrations in the outer cannula and then through the vocal cords, allowing phonation (speaking), clearing of secretions, and more normal breathing through the mouth and nose. This can assist in the weaning process from ventilatory support. When the cuff is inflated, the tube seals the trachea, helping with positive pressure ventilation and protection from aspiration. The internal diameter of 7.6 mm allows relatively low airflow resistance, and the overall length (81 mm) ensures that the distal tip is positioned appropriately in adult tracheas without being overly long. The cuff resting diameter of 27 mm provides a sealing surface while trying to minimize pressure on tracheal mucosa. 8FEN
The reusable inner cannula is a key feature for hygiene, convenience, and safety. Secretions often accumulate in the lumen of tracheostomy tubes, which can lead to obstruction or increased work of breathing. By having an inner cannula that can be removed, cleaned, and replaced (up to the recommended duration), care teams can maintain patency of the airway without needing to remove the entire tracheostomy tube. The twist‑lock 15 mm connector ensures compatibility with standard connectors and airway equipment. The included obturator (a rounded, smooth insertion guide) helps facilitate safer insertion at the time of tube changes. In addition, the decannulation plug allows temporarily closing off the inner cannula’s lumen to allow airflow through the fenestration and upper airway, which is useful in weaning or when trials of “capping” are desired.
Clinically, the 8FEN model is chosen in situations where the patient has a need for mechanical ventilation or airway protection at some times, but also where phonation, cough effectiveness, secretion management, and upper airway function are goals. It’s a more “versatile” tube: you can switch between sealed (cuffed) ventilation and more natural breathing / speaking phases by deflating the cuff and/or using the decannulation plug. However, there are trade‑offs. Because it is a large tube (O.D. 12.2 mm) it carries risk of pressure on tracheal walls, risk of injury if not properly positioned, risk of granulation tissue formation, etc. Cuff pressure must be carefully managed to avoid ischemia of tracheal mucosa. Also, fenestrations introduce risk of secretions getting caught or obstructing the fenestrations or inner cannula; care and frequent cleaning are essential. Finally, selecting the correct size (both diameter and length) is critical so that the tube works effectively and comfortably with minimal complications.
