The Femcare AVM-851J Filshie Clip is a surgical implant designed to occlude (seal shut) a portion of the fallopian tube, thereby preventing the passage of eggs and providing sterilization without removing or cutting the tube. The clip utilises a titanium outer shell, which is radio-opaque (thus visible on imaging), combined with an internal silicone lining that compresses the tube lumen. The titanium-silicone combination allows the clip to maintain mechanical closure while minimizing tissue trauma and permitting biologic sealing over time.

In surgical use, the clip is applied via a specialized applicator (Filshie applicator) in a laparoscopic or minimally invasive approach. The procedure is relatively quick and mechanical: once the tube is properly positioned in the jaws of the applicator, the clip is compressed until a mechanical “stop” is reached, locking it in place. The silicone lining helps ensure that the tube lumen is reliably occluded while preserving the surrounding tissue. Unlike surgical resection or cauterization, the Filshie approach avoids cutting or removing tissue and is often considered to have less operative morbidity and potentially easier reversal (in some cases) because the tube remains anatomically intact around the clip site.

Over time, the clipped area undergoes avascular necrosis (i.e. the tube tissue compressed under the clip loses blood supply), sealing the occluded section permanently. Because the clip does not release hormones, metals, or drugs, it is considered a non-hormonal, permanent contraceptive option. The design includes safety considerations — for example, the IFU states that if there is any doubt about clip placement, a second clip may be placed adjacent to the first. Additionally, the device is intended only for use by qualified physicians and carries restrictions in labeling (e.g. “U.S. Federal law restricts this device to sale by or on the order of a physician”).
