The Centered ALTO Total #605 is an ossicular prosthesis designed for total ossicular chain reconstruction (i.e. replacing the entirety of the ossicular chain in the middle ear) when the native ossicles are not usable (due to disease, trauma, removal, or congenital absence). It is part of the ALTO family from Grace Medical, which emphasizes adjustable prostheses. In this model, the shaft has a telescoping or adjustable design, allowing the surgeon to fine‑tune the length between about 3.00 mm up to 7.00 mm in situ, to accommodate varying anatomical distances between the tympanic membrane (or reconstructed eardrum) and the stapes footplate. The “centered” designation means its geometry is aligned so that the prosthesis is aimed to sit centrally on the stapes footplate, reducing risks of off‑center contact and optimizing sound conduction. The head of the prosthesis (3.00 mm) provides sufficient surface area for coupling with the tympanic membrane or graft material.
