The 758-450 Megerian SRP is a stapes replacement prosthesis designed for use in stapedectomy or stapedotomy procedures, particularly in cases where the native stapes has been removed or is nonfunctional. Its distinguishing innovation is the use of nitinol shape memory alloy to enable a heat-activated, self-crimping mechanism around the incus or remnant ossicular structure. This design obviates (or substantially reduces) the need for the surgeon to manually crimp or bend the prosthesis during placement, which can reduce manipulation trauma, variability in crimp tightness, and risk of incus damage.

Structurally, the prosthesis incorporates multiple tapered nitinol arms (e.g. six in many depictions) that are designed to conform naturally around the incus upon activation, adapting to slight anatomical variations and providing stable retention. The shaft is relatively fine (often ~0.6 mm in analogous models), and PTFE or fluoroplastic elements (such as a polished or insulating surface) may be included for smooth gliding interaction, minimizing friction or tissue trauma. Because of its self‑adjusting wrap behavior, the Megerian SRP is particularly suitable in revision surgeries or in anatomical settings where standard rigid prostheses are difficult to crimp or seat reliably.

From a clinical perspective, the Megerian SRP aims to offer both ease of surgical handling and stability of coupling, potentially improving consistency in postoperative hearing outcomes. Its self-crimping mechanism helps standardize the incus–prosthesis interface, and the shape memory characteristic promises a more predictable and gentle grip. In revision or complex cases, its adaptability can be beneficial in managing irregular incus remnants or compromised anatomy. Because the device uses metallic (nitinol) components, care must be taken regarding MRI compatibility and heating, though Grace Medical designates the SRP family as MR Conditional under specific limits.
