The Synthes 323.023 Wire Sleeve is a surgical guide component designed to be used with 1.6 mm guide wires (K‑wires) as part of small fragment LCP (Locking Compression Plate) systems. Its purpose is to protect the surrounding tissue and to ensure accurate alignment during drilling or insertion steps that involve the guide wire. When you place a small guide wire through a bone fragment or across fracture lines, this sleeve slides over the wire and sits in a plate hole (or drill guide) so that subsequent drilling or screw insertion proceeds with minimal deviation.

This wire sleeve plays a supporting but important role in minimizing errors in trajectory. By providing a stable channel for instruments over the wire, it reduces the risk of wire bending, skewing, or movement during drilling or when transitioning to larger bits or sleeves. It helps especially in anatomical areas with limited space or visibility—small bones, near joints, or in regions with soft tissue that may displace or tug on the wires.

From a clinical standpoint, the 323.023 helps improve the precision of surgical fixation in small fragment trauma or reconstructive orthopedic cases. When used properly, it can reduce malalignment, decrease operating time (less re‑adjustment), decrease risk of soft tissue injury, and improve implant fit. However, critical factors include ensuring the sleeve is clean, free of burrs, that it fits properly over the wire (no excessive play), and that wires are placed correctly before sleeve insertion. Also, wear over time of the sleeve bore (inner diameter) or damage to its distal end can degrade performance and lead to increased deviation or wire bending.
