The Shiley 3.0PEF cuffless pediatric tracheostomy tube is designed for children who require a stable airway but do not need an inflatable cuff. Its 3.0 mm internal diameter is suitable for smaller pediatric airways, and with an outer diameter of about 4.8 mm and a length of 39 mm, it is sized so that the distal end reaches an appropriate depth in the trachea without excessive protrusion. The cuffless design allows airflow around the outside of the tube, which can facilitate more natural breathing, better phonation (if the patient has that functional capacity), and lower risk of pressure injury associated with cuffs.

The tube’s construction and design features aim to enhance comfort, reduce tissue trauma, and improve ease of use. The soft, transparent flange allows caregivers and clinicians to visually monitor the stoma and surrounding skin for redness, breakdown, or other changes. Design elements like the comfort recess (reducing pressure at the lower edge of the connector) and a tape‑tie recess in the flange help secure the tube while minimizing contact irritation. Also, the material uses non‑DEHP plasticizer, a feature increasingly preferred for pediatric devices to reduce potential risks associated with certain phthalates.

From a clinical standpoint, the 3.0PEF cuffless tube is best used when positive pressure ventilation requirements are modest and when protecting the airway from aspiration is less of a concern (or other protections are in place). Because it does not have a cuff, it cannot provide a tight seal, so leaks are expected and must be managed (in ventilator settings, humidification, etc.). It may be more comfortable for long‑term use in non‑ventilated or spontaneously breathing pediatric patients. Frequent monitoring of fit, stoma condition, sputum clearance, and proper fixation are all important to avoid complications such as skin breakdown, accidental decannulation, or granulation tissue.
