The Shiley 4FEN tracheostomy tube is designed primarily for adult patients who require a secure airway with the possibility of phonation or breathing through the upper airway. The “fenestrated” design includes holes in the outer cannula which, when the inner cannula is removed or bypassed (and when the cuff is appropriately deflated), permit air to flow upward through the vocal cords, enabling speech or more “natural” breathing. The tube’s cuffed configuration (low pressure, high volume) allows clinicians to seal the airway during mechanical ventilation or when protection from aspiration is needed, reducing air leak and improving ventilation efficiency. The Size 4 has a 5.0 mm internal diameter and ~65 mm length, appropriate for many adult patients with moderate airway size. The cuff resting diameter for this size is about 20 mm.
The presence of an inner cannula is an important feature: in the “FEN” model, the inner cannula is reusable and engages with a twist‑lock 15 mm connector. The inner cannula helps in maintaining lumen patency (important when secretions accumulate), allows cleaning without removing the whole tube, and improves safety by ensuring that the tube’s lumen is not obstructed. The outer flared (flange) component is soft, opaque, and swivels, helping reduce skin irritation and allowing some movement without disturbing the securement, while maintaining stability. Medtronic’s materials are latex‑free, minimizing risk of allergic reactions in sensitive patients.
Clinically, the 4FEN tube is particularly useful when there is a desire to transition a patient toward speaking, or to permit upper airway airflow during weaning from ventilator support. However, these benefits come with careful management: the cuff must be deflated for the fenestrations to be effective (but this may allow leak and require adjustments in ventilation), and frequent cleaning of the inner cannula is necessary to avoid blockages. Also, because a reusable inner cannula carries risk of buildup of secretions or biofilm over time, protocols for cleaning (duration, frequency) are essential. Patients must be monitored for cuff pressure, stoma care, cuff fit, and for obstruction of the airway or inner/outer cannula. The dimensions (size, length, diameter) must be appropriate for the patient's anatomy to avoid undue pressure, trauma, or risk of accidental decannulation.
