The Synthes 312.35 Double Drill Sleeve 2.7/1.25 mm is a precision instrument used in orthopedic and trauma surgery during the process of inserting 3.5 mm cannulated screws. It has two ends (or two sleeves) of different inner diameters: one end sized to guide a 1.25 mm guide wire (or K‑wire) and the other sized to accept the 2.7 mm cannulated drill bit. The sleeve helps maintain precise alignment of the drill bit with the guide wire and screw hole, which is critical for correct screw trajectory, proper screw purchase, and minimizing damage to surrounding soft tissues or cortex.

Using a drill sleeve like this also helps protect soft tissues (muscle, periosteum) and reduces "wandering" of the drill or guide wire, which can lead to misdrilled holes or weakened fixation. Because it is double‑ended, the surgeon can quickly switch between drilling with the smaller wire and then the drill bit without changing sleeves or instruments, which helps efficiency in the operating room. The guide‑wire sleeve end ensures that the guide wire stays stable during drilling, which is especially important in deeper or less accessible anatomical locations.

From a practical standpoint, the 312.35 sleeve must be used with care: ensure that the sleeves are clean, free of burrs, and that the inner diameters are not worn (which could allow play). Precise alignment of the guide wire before using the 2.7 mm drill bit sleeve is essential, because any misalignment upstream will be magnified downstream. Also, selection of the correct sleeve for the size of screw and drill bit being used is necessary to avoid mismatch. Proper sterilization is required, as is periodic inspection for wear or damage. When used correctly, this sleeve contributes to stronger fixation, fewer complications from misplacement, and smoother surgical workflow.
