The 4 mm Cancellous Driver is an instrument used in orthopedic surgery for inserting or removing cancellous screws of 4.0 mm diameter. Cancellous screws have coarse threads and are used in softer, more porous bone (like metaphyseal regions, cancellous bone). A driver of this size will have a tip tailored to the screw head type (often hex or proprietary Synthes socket) and must deliver sufficient torque to seat the screw securely without stripping the head or damaging bone. The somewhat longer length (≈ 11 inches) is beneficial for reaching screws placed deeply (e.g. in pelvis, proximal femur, or when working through retractors) or when working in minimally invasive or constrained exposures.

Because of its role, this 387.277 driver must balance rigidity (so you don’t lose torque or squirm under load) with manageability (so you can maneuver it in tight spaces without hand fatigue).
